Osmotic pressure. Alcohol will osmose through a cell wall, increasing the pressure until the cell wall fails.

Alcohol is a highly permeable substance to your cell membrane. It would make your cell burst or become apoptotic only when enough has accumulated. That is for topical use. For the time when you consume alcohol it also has the same effect but on a much more massive level, simply becasue it is being transported to every area of your body via your bloodstream. So then it causes the release of endorphins and other depressive effects, but thats a different story.
