The Milkyway is a galaxy which is essentially a cloud of stars dust and gas. Even though on our terms the stars in this cloud are very far apart they have enough gravity to pull against each other and as a result the cloud has a center where the stars are very close together and then the stars get further apart as you travel toward the edge of the galaxy. Also, our galaxy spins which has flattened it into a disk shape. So we live about two thirds of the way from the center of a spinning disk of stars with lots of loose dust and gas clouds mixed in. Kind of like a fluffy candy with stars playing the role of nuts in the round candy patty.

omigod all the answers are wrong!

the truth is, we really dont know. what follows is a calculated guess by scientists.

the milky way has a massive, massive black hole in its centre. when the big bang happened, moving particles got attracted by its gravity and began revolving around it.
also, now scientists think that WIMP(weakly interactive massive particles) and dark matter is at the edge of the galaxy, which makes the whole thing stretch out a bit.
the matter in the galaxy is between the combined,op posit gravity fields of thee dark matter and th black hole.

it's all guessing, but this is our best guess.

there's a inactive massive black hole in the centre of the milky way wich holds it together. moreover the gravitational pull of the objects in the milky way also puts it together and these both keep in hold milky way's satellite galaxies-large magellanic cloud(LMC) and small magellanic cloud(SMC)
the black hole in the centre was created during the birth of the milky way wich kept all the objects together
