If you live in the U.S., you get the application for a passport at the post office. It will inform you what other documents you need (one will be your birth certificate) and you will also need 2 passport photos (these range from free...if you are a AAA member and have them take the photos...up to $15). You take everything back to the post office and they will start the processing. Everything then gets mailed to the closest passport office. The cost for the passport itself will be $65.

Most US Postal offices have passport clerks, and forms you can fill out. I think they are $68 in the US and take about 6-8 weeks to process once you get photos taken and submit your birth certificate and such to get it processed.
