Some do and some don't.
The difference is in its execution. Some people are perfectly contented in that frame of mind. Their horror is a gift that they share with others to make them appreciate their lives and how precious they are.
However, some are mentally ill and they lose the boundaries that their mind has between fantasy and reality. This, more often than not, occurs with people who watch horror movies and not necessarily people that make them.

No, there just out to make a buck. It's the people who go to see them that have the sick minds. However, I think they're greedy and immoral, but not sick.
