Burried alive in a coffin because you stay alive for days and can barely move. There is no way you will dig yourself out from six feet of dirt and no one will hear you scream. You will be begging for death if that happened.
I've thought about this one and if I was ever in that situation I would bite myself and make myself bleed 'till I passed out. Gross bit of thinking, I know.

Spiritually speaking... I think a quick and unexpected death would suck. I know it sounds a little shady, but if given a long and drawn out death then you can make amends with all possible deities out there and have a little more of a fighting chance at a decent seat in eternity.
Physically speaking... You ever hear about that guy that had the boulder fall on his arm and he had to saw it off to live? What if he had just stayed there and died? I think dehydration, starvation, blood-loss, infection, and isolation would be a pretty bad way to go. But that's just me.

The worst way to go surely is to survice only just, then again go through the death process and keep on edge of death for a good while like that. So I'd say the worst way to go is very very very slowly.

Suicide due to Terminal Boredom. Being bored to suicide while locked in a room as someone elses auntie blathers on about their recent surgery, their kids' stint in rehab, them finding jesus, allah or their extra car keys...While the plain white room has no windows and the only show the TV will play is "the View". Hell, by the first commercial break youll be wishing for a nice cyanide capsule.

Skin Cancer.I worked at this nursing home which we had this patient that had it all on the upper part of her body. There was no skin left on her head and total skull exposed, nose rotted away,no lips left.Flies wouldnt stay off of her. That was probably the worst way I have ever seen someone die.

In lots of pain for a long long time, not days but years. My mother had a lung problem and it lasted for 3 years, wasn't cancer but was very painful, near the end she just wanted to die and end the pain. It was hard on me, but I was glad for her when she died because I knew the pain she was in.

Being burned alive or drowning....to me those would be the two most painful ways to die.

I would say some type of painfull chronic deases, one that lasts for months or years... I think that is worse than being burned of drowning.
Edit: Or what the person above me said
