It means a woman who's never given birth. Abortions and miscarriages BEFORE the 20th week do not count in determining parity [for example, a woman who's been pregnant 3 times but whose pregnancies ended at any time, by any means before the 20th week of gestation is considered a nullipara]. This woman would be Grava3-Para0-Ab3. Any abortions or stillbirths from the 20th week onwards DO count as a birth each. So, grava and para not necessarily always go together.
