Heres a thought how about a small toy for baby and the day off for granny or even a cooked meal?? A new robe for her something...She probably feels unappreciated..Or how about taking her for a professional portrait? That would be nice..Include the grandma...She'll thank you for it...

That depends on what she already has. My daughter is 5 1/2 months old and at 4 months she really started to enjoy an activity mat. She is able to lay on her back and there are toys that dangle overhead that she is able to reach and play with. The toys also come down and attach to the mat for tummy time. Also, a doorway jumper or exersaucer would be a great idea. Any toy that is easy to grasp and has different textures is good too. Ask the grandmother what she needs. If she already has tons of toys, maybe you can just pick out a cute outfit. My daughter has tons of clothes, but I always enjoy receiving another cute outfit!

My mum brought our baby home this nifty floor mat the baby can lay on and look up and play with a little set of toys hanging above - you can also take off the hanging toys and put them over the crib.
This is good for 0 - 5 months to get them to look up, reach up, and when they get hold of the toys they crinkle or one is a mirror the baby can look at itself in. It's also a nice soft place for baby to lay in the floor, and after 5 months (or really, when the baby starts pulling up from objects) you take off the toys and it's a little tummy mat for the baby. My little one's pretty much a newborn, but he already is showing some interest in the bright colours when he's awake. Your baby cousin would probably like that, I should think.
The diapers and practical stuff would be nice for Gran, but you did say for the baby.

I know this sounds a little boring, but...DIAPERS! I wish someone would've bought more diapers for my babies. But also look at the lables on toys in the store. They all should say what's age apropriate. It's always fun to buy lots of clothes for little girls. My children's godmother is a fan of baby jewelry and I love to see little girls in jewelry.
I wouldn't buy too much at this age, though. You can always spoil her by just being there and playing with her. Later on in life that's all that would matter anyway

Honestly, she a little young to care what you buy her. Babies are not materialistic. She will know the love she is given by her family. Get her stuff she needs like clothes! Babies grow out of clothes really fast. I say buy her a cute outfit.

I have a baby niece and I always buy her GAP baby stuff like shoes, dress, a hat, a robe (they really liked the robe), pajamas etc... you can also get that at Target. I also painted her a painting with her name in it. so you can get her something for the room like a picture, a wind chime, something decorative. You can get her a blanket or a pillow. a puppet is cool too.
Hope this gives you ideas....

An exersaucer! It's a great toy starting at that age. They are a little pricy but well worth it. A doorway jumper is a fun toy too.

As a mom myself, the best gift anyone could have given me was diapers, wipes, and infant formula! Clothes are easy, so diapers, wipes, and formula is the best!

4-month-olds don't really "enjoy" much... anything you get her will be appreciated by her grandma more....
Onesies- those little undershirts that snap between the legs- those are super-handy. They keep that little tummy covered, and now, they come in some pretty cute prints and styles.
