Go to your county clerk office and get one. But you will still need to pay a filing fee to the county clerk and then you will have to publish your fictitious business name statement in a local newspaper and pay for that too.

When I started my business, I went to City Hall. They gave me all the forms I needed as part of getting my business license and DBA was one of them. It still cost $40 but that may be different where you live. Good luck with your business! I hope you make lots of $$$!

Either call your dept of commerce and have them guide you to the website with the form, or search for it yourself on the Internet.
