Whenever they wear out in the waist, legs or anywhere else or they are stained. It depends on how often you wear them really. The rule of thumb is: if you think you need to replace them, then you do.

I think many things come into play when deciding when to replace underwear.
How many do you have, what types, are you in a relationship, do you hand wash or machine wash.
Depending on answers to the above the time should be shorter or longer. For example... If you are in a relationship and only have a few pairs of underwear, you will want to replace them more often to keep everything looking fresh and not create bad or negative energy in the relationship. If you hand wash they may last longer than if washed by machine.
It is all up to you.

Basically whenever they start looking or feeling NOT as good as new. In other words...
Looks:
Loose fabric, bunchy
Waistband stretched out so it doesn't fit
Stains
Holes, runs, tears
Feel:
Pilly
Not soft
thinning
If any of these are happening then it is necessary to replace that pair. Usually people buy all their undies at once so if one pair is like this then probably all the other pairs are (or are close to) like this.
The often part really depends on how rough you are on your undies.

Depends on how many times you wear them, for example I think a man should only wear his underwear a maximum of 10 washings! I hate underwear that are too losen for too much washing. It doesn't mean you should keep them on dirty so you don't get to wash them too often LOL

As soon as you see a small hole or thinning in the fabric, throw them out! If you have stains that won't wash out (eeyoo), through them out! If you are going to a wedding, special event or plan on a very romantic evening, buy new undies! Always play it safe!
