You start by buying flowers and not roses (there so cliche), then set up reservation at a nice restaurant or make plans to take you intended out to a nice place to eat. A movie is always a good choice to get closer and start a conversation after. Close with a walk if the weather permits and a handshake or kiss on the check to show you are a gentleman. She'll be begging to see you again, soon.

well, first you have to ask someone to go out with you. call the girl (im guessing you are a guy) and ask if she would like go out with you.
for the date, do something that the date-e likes to do. make sure that you make reservations if you need to and then, be yourself!
