APA formatting is usually at the college level.

For elementary school, it depends on what kind of report you are writing. If it is a book report, you will want to include the heading, (name, date, room number, school), the title, the author, the characters, the setting, and what the story is about in a brief summary.
In other word, follow the 5 w's: (who, what, when, where, and why). Try to be as descriptive as possible, so as to interest the reader.

If you are writing a composition, it depends on what kind.: narrative, expository, persuasive, descriptive, or creative.

Narratives desribe a true event as you have witnessed it. Explain each part in sequence.

Expository writing involves your topic, and at least 3 points to expound on in that topic, with an explanation of what you mean for each point, and an example for each explanation to draw from.

Persuasive writing involves trying to convince another of your point of view.
It involves choosing a point of view, with 3 reasons why you think this, an explanation of each reason, and an example of each explanation.

Descriptive is simply describing things using the 5 senses.

Creative is simply a made up story. (5w's)

I hope this helps, I'm really not clear on what type of report or school to which you are referring. This makes a difference.
