It's actually a number for the trainer to remind him of how many ml of steroids they are taken in their cycle. i.e.
smaller guy : Aaron Boone #17 = .17ml
bigger guy: Travis Hafner #48 = .48ml
*****note****I am not saying these players are taking steroids they are only being used theoretically

there is no set way now, only tradtion, like pitchers dony usully wear lower than 10. they used to be given out to people in the order they would bat, hence the pitchers being 10 or above..
