Oh yes, you better believe it. The NSA and Britains GCHQ both monitor and record all Internet activity. In the main they simply log the sites you access and take no notice unless some reason comes up like suspected terrorist links. Some sites such as those with terrorist links, access to arms, explosive constituents and the like are flagged and your use of them would be sent to an intelligence assessor.
For years NSA and GCHQ have had the ability to electronically monitor all phone calls virtually anywhere in the world through a program called Echelon. A filter looks for certain words and if it detects them it then records the conversation and again sends it for analysis. A lot of people are convinced that the same technology is routinely used to monitor emails.
Nothing to worry about for an innocent person.
Great strides are being made by police forces for instance in tracking down users of paedophile sites and that can only be a very good thing.

I am a systems administrator with UNC-CH, so I'll try to give you a little direction in this area, but not a complete answer.
Can you be completely anonymous on the internet? It's not easy, but it also depends on who you wish to stay anonymous from.
An email, for example, tends to include all the steps the message took from origin to destination, though there are methods for faking that information in the headers. You normally don't see the headers, but you can change your email client preferences to allow you to see them. There are also numerous ways that you can trick someone into revealing their location such as by dropping a 1 pixel image into an email. If your email is set to allow remote images to display, I could trace you when you open the email because your computer will connect to my web server to retrieve the image. That will give me the IP address from which connected (generally the IP address of your cable, dsl, or dial-up modem). That's usually enough by itself for me to locate you to within a certain geographic area.
For someone such as a student trying to hide their internet activities from their parents or an employee trying to hide from their boss, it is MUCH more difficult to hide activity. If someone has access to your computer physically, you can still takes steps to stay more private, but complete privacy is nearly impossible. In a corporate environment don't bother: they control the connections to your computer, so they can see every data packet you send or receive. In a home situation, there are also NUMEROUS tools available to parents that can be used to monitor activity.
So the answer to your question really is that the ability to track or be tracked depends on the situation, particularly the location from which your are operating, the skill level of the person seeking to avoid detection, and the skill level of the person trying to discover information, much as in the case of the "real world" equivalent where one person is trying to avoid being found by another.

There are currently government filters in place that track internet access by using the unique IP address of your internet connection. They scan for users visiting militant or terrorist web sites and those visiting porn sites to be sure people aren't looking at kiddie porn.

In most cases you can be tracked by the unique IP address that is assigned to your location. There are ways around that, but most law-abiding people don't need to know that stuff...

yes! even if u delete your history it is all still on your harddrive and u would have to completely destroy the hardrive to cover your track but i guess that is bad cus then u wouldn't be able to use your computer.
