Like author Lauren Weisberger of The Devil Wears Prada? Do they get a flat fee? Percentage of sales?

The author will hopefully have publishing rights and is being paid for Royalties. Whatever that percentage is, is going to be the percent he or she makes from the movie. It's much better to be paid from royalties than off of one lump sum because the author will continue to be paid as long as the movie is being sold.

it really varies - whatever their agent can get for them.
a REALLY popular book like DaVinci Code can get millions, plus percentage of box office sales, plus percentage of merchandizing, etc.

a not very well known book will get a lot less. Phillip K Dick sold the rights to Blade Runner for only a few thousand dollars.

depends on the popularity of the book --- the authors I have heard on tv said they got --- a set amount upfront and then they collected a certain percentage of the money the movie makes

It depends on the contract, how much the company is willing to pay the author, and bunches of other things.
