Well Top Gear can mean a number of things - most usually the higest level of performance for a car or bike, because a gear is an element that helps the machine run at a certain level, so Top Gear is a mark of top performance.
Gear is also a generic name for "things or clothes" You can wear leather gear with protective headgear while riding your bike in top gear, and listening to your favourite songs on your new iPod gear.
So - something that signifies levels of performance, or clothes or things. Gear.

This is usually used in regard to a vehicle of some sort. Especially a car or a bicycle.
When the car's transmission is manually operated, you push in the clutch and use the stick shift to change the "gear"
When you have shifted the farthest you can, you will be in "top gear"
On a bicycle that has little levers near the handlebars, you can change the ratio that the pedal turns the rear wheel by way of shifting the "gear". When a bicyclist is pedaling the bicycle with the maximum tension and highest ratio, he will be in "top gear".
