It means you should be off to see a doctor ..pronto...or if it is someone you know..make the appointment and get them there now. They will need a specimen of the product being coughed up for analysis and they will need a blood workup to see if their haemaglobin count is low. Haemaglobin is a part of the red blood cells. As we breathe in oxygen the red blood cells pass the lungs and the oxygen attatches to the haemaglobin on them. If there is loss of blood due to whatever factor (coughing up blood in this case) then there will be less oxygen taken around the body. Therefore, you or your friend will feel fatigued (lethargic or tired).
Coughing up blood can be from a number of factors...one is from too much coughing that is aggravating the airways..and another is a TB (tubeculosis)..perhaps the person is not immunised against this? Something else is lung cancer, that can be eliminated through diagnostics of the specimen you provide.
IN any case..get to the doctor or ER, they can do alot to help treat the symptoms whatever the case. And allay your fears of what might be...also, make you better :)

How much blood? What color is it? How often? Where is it coming from, your lungs or your stomach?

Depending on the answers, I don't know that you need to drop what you're doing and run to the ER. If you're having difficulty breathing, coughing up or vomiting bright red blood, or are really in a lot of pain, go to the ER immediately. Otherwise, you could have anything from a viral illness to a problem with your digestive system. See your doctor. Something like this needs to be treated, but not urgently.

As you mentioned that you were tired. This can lead to dehyderation and consequently dry throat. This dryness can casue unsticking of throat mocusa and when you coughed, this layer of epithilium erupted and also have some blood alongwith. Till here, there is no worry.

Second thing: If you are having mild to moderate fever for last couple of months and also loosing weight then you need to have some follow up. You should go to Lab and have your AFB test, this will confirm TB or something ( God Forbid). If you are not having above symptoms, then ignore second comments.

It could mean several things, but it definitely means you need to see a doctor. Call a doctor's office and tell the nursing staff your symptoms SOON, like tomorrow.
