Most people in the T community use one of the following terms:
transvestite, transgender and transsexual

These terms are used to refer to various degrees of severity of what is currently referred to as "gender identity disorder"

To greatly oversimplify a transvestite is a person who puts on clothes of the opposite birth sex, but does not feel a need to identify with the pscyho-social role of that sex.
Such "cross-dressing" may be associated with sexual gratification.

A transgender person is a person in whom a degree of cross-dressing is associated with a need to identify with the psycho-social role of the opposite birth sex.

A transsexual person is a person in whom the gender identity disorder is so acute that he/she wishes to identify fully with the psycho-social role of the opposite birth sex.
Numerous transsexuals ultimately make a full physical transition (involving SRS - sexual reassignment surgery) so that they can realize what they feel is their true potential.

A person who is born with the physical traits of both sexes is intersexed.
These cases can be particularly tragic. I know a woman who was born with a complete set of ovaries as well as a penis and testicles. Her condition was not recognized at birth and was only discovered following a physical examination prompted by a suicide attempt in her thirties.

I consider myself to be a transgender person.

It is when a person has been born with ambiguous genitalia, and the doctors need to perform tests to determine if the child is more male or female. The variations are great; it is important to not look simply at external reproductive organs, but internal ones as well. A person who is intersex most likely has a bit of a rough life; I can't imagine how difficult it must be.

In simple terms, a person who has the mind and feelings of one gender, but the body of the other gender.

Imagine if you woke up tomorrow feeling exactly the same as you do now, but got out of bed and found your body was that of the opposite sex - would you be happy?

I think it must be one of the most devastating things, particularly at puberty where a person's 'real' (but not true to them) gender is 'confirmed'.

Although the first answer given is technically correct, the term "Transgendered" can be more easily understand to be a person who has changed themselves either superficially through clothing (transvestite) and make up etc, or through surgery (transsexual) in order to become a member of the opposite sex to their stated gender.

I.e. A bloke trying to be a woman
or A Woman trying to be a bloke.

A person who has both male and female genetalia (sexual organs) is an Hermaphrodite.

Correlation's answer is more someone who is intersexed or chimeric. Someone who is transgendered is someone (like me) who is thinks their mind/soul was born into the body of the wrong gender but that person has not surgically or hormonally altered his/herself (then that person would be a transsexual).
