For now birth certificate and ID. As of December 2006 you will need a passport.

If you are under 18 years of age usually the border patrol will require a birth certificate and a written note from your parents with their signatures, names, address, and phone number. The note must also be dated and state that your parents know specifically where in Mexico you are headed, who you are with, and how long you'll be gone. A passport would be a better alternative to the birth certificate but the note is still required either way. If you don't they can hold you up at the border for hours or days trying to get in or out of the U.S. Have a fun trip! :)

Go to your local authorities (town hall, etc.) and ask them what the exact wording should be on this affidavit, if I am not mistaken, I think you also have to have it notarised.
They will probably tell you where to have this done.

If you're under 18 and a citizen of the United States, take your birth certificate but let your friend's parents carry it for you. If you are a resident alien, you need your resident alien card (green card)
