go to the dmv and procure another one. depending on where you live, you may have to pay for the replacement. you can always call the department in advance and get the entire run down that way.

Contact the DMV. Preferably by phone or in person as if you do it online, it may take some time. You'll need to complete a form and bring proof of identity. However, if it was stolen because of a crime, you'll need to complete a form that is only available from the police department. Bring that form and the form they have at the DMV in with your proof of identity.

It depends, did someone in a blue uniform take it from you, or did you just physically lose it? In some states, you can order a replacement online from the DMV. I had to do this once before.

I would report it to the DMV where you live right away. You can always search for your DMV in your area by google it by keyword reporting lost driver license, "your state" DMV, ect...

First make a police report. Then go to the DMV (department of motor vehicles) and request a new one. Which ofcourse will cost you money to replace it. Good Luck.

You need 5 proofs of who you are...passport, birth certificate, bills in your name to your address, etc. Then just go down to the DMV for a dulpicate license.
