Yeah. Our conscience, memory and thinking ability depends on our brain being in working condition. If damage to our brain can cause memory loss, interuptions to our thinking processes or cause us to lose consciousness, then what would death do?

I personally do not believe so, but then again, you are basically addressing the age-old question of life after death...
Everyone
Everyone's got a theory here, but I would submit that perhaps a portion of your awareness is carried forward, but not all. I would think that what is known as our 'personality' is stripped away, while perhaps memories and other life-lessons are retained, to be assimilated into the mill...
The union of souls that awaits us has no need for an ego, or any related pieces of 'self'. The muck that is determined useless is simply caught in the mesh, and perhaps lingers in the cases of those unwilling to let go... That's where we get ghosts from :)

This question has so many answers, depending on who you ask. Many religions believe in an afterlife, so they believe that the consciousness lives on (your soul). But coming from a scientific point of view, when you die, your brain functions stop. You, as a human with a mind and consciousness cease to exist. So really, it's all in what you believe.
