Not an NHL game but a pro game:

Hundreds of Latvian hockey fans, officials and players turned out Monday to pay their last respects to Sergei Zholtok, the longtime NHL and Latvian national team player who died during a game last week in Belarus.

The 31-year-old Nashville Predators forward died of heart failure about five minutes before the end of a game last week between Riga 2000, the Latvian team he was playing for during the NHL lockout, and Dinamo Minsk.

Then there was the fan who died:

Brittanie Cecil, 13, was struck during a game between the Columbus Blue Jackets and Calgary Flames at Nationwide Arena. She was taken to Children's Hospital in Columbus and died Monday.

Bill Masterton of the Minnesota North Stars is the first and, so far, only player to die in an NHL game. On January 14, 1968, He tripped, fell backwards on the ice, and hit his head hard causing it to bleed profusely (he was not wearing a helmet). He died the next day.

The Masterton trophy is named in his honour, and is given yearly to the player who displays sportsmanship, dedication, and perseverance

I don't know off the top of my head.....
but there was that time that Chris Pronger got that slapshot in the chest and his heart did something wonky.
And just last season some guy has a seizure on the bench.

Howie Morenz sort of died from playing hockey, he broke his leg and couldnt play any more, he died about a day later, some say due to a broken heart cause he couldnt play hockey any more

A kid died in my home town by getting a puck to the back of the neck. He was 13. Other than that, I'm sure there have been others but I don't really know.

yes. someone did. sorry can't give you the name cause i can't remember.
but this is what happened: the guy was tripped, or fell accidentally, hitting his head with the ice (and nobody wore helmets back then), then unfortunely he passed away.

i heard that during training died player i don't remember his name,
accidentally his throat was cut
