Try cleaners like CLR or Lime Away you can get them at Wal Mart or any store like that. I had good luck using them on rust stains.

Oxalic acid. Zep deck and fence cleaner concentrate (read label make sure it has oxalic acid). Home Depot has it in Paint Department.
Add one cup to bowl and one cup to tank. Let it sit for an hour. Use a small brush to clean the tank insides. Flush toilet and hold the handle down let the water run continuously for one minute. If stains remain above the water line use ZUD scouring powder and small brush to remove.
Use this treatment once a month. Only use ONE CUP this is powerful stuff!!! Wear gloves and eye protection.
You can also use Zep DILUTED with water to remove rust from sinks, showers, shower curtains, counter tops etc...

I heard if you out coke-cola in the bowl and let it sit, it'll clean rust stains. If that doesn't work, I've used Mr. Clean's Magic Eraser in mine. Just open the tank and hold the balloon up so the bowl is empty and you can scrub it well.

Drop a can of Coke (classic) inside of the bowl. Brush and rinse all metalic parts with it too. You won't believe your eyes when they see how clean they can become. Just be careful not to leave it there for more than 3 hours, or it'll eat your thing up.

open a can of coke, drop it in the toilet for a few hours and then using a brush go around the toilet...of couse remove the can and flush...this really works! its amazing!

SANI FLUSH gets all the stains out... but be careful it is reaql poisonous. Use and store safely.

If it is really tough, you can purchase a PUMICE STONE, requires a little work but, it will do the trick.
