I vote for Nicholas Cage too, but for that painful Italian accent he attempted in Captain Corelli's Mandolin. Put me off the whole film.

Didn't Angelina Jolie do a British accent for Tomb Raider? At least I think it was supposed to be British...that was awful. I completely agree with you about Val Kilmer in The Saint, it was embarrassing to watch wasn't it? Also, Shannon Elizabeth's accent in American Pie 1 & 2 was bad enough to get an honorable mention...but maybe when you're that hot you can get away with it.

The kid who starred in the movie of Peter Pan sounds like he's tried to do an English accent, but failed. And I think Dick Van Dyke's accent in Mary Poppins is more than made up for by his performance.

I think Nicole Kidman (Aussie not Brit) slaughtered a southern accent in Cold Mountain along with her co-star Jude Law (Brit).
I am not sure what accent Hallie Berry was going for in the first X-Men as Storm, but what ever it was it sucked.
I don't think Liam Nee son can pull of an American accent without giving himself away.
My absolute favorite horrid accent attempt is Steve Martin's fake French accent.
I wish I could give you some more but it is really freaking early and I am not awake yet.

I think the majority of British people can do an American accent, I myself can do a rather good impression. So, I can't really think of any actors that sound bad doing an American accent.
I think what people get wrong in movies when they try to do an English accent is they either do it too posh, or they try and do cockney, and it just doesn't work. I think Angelina Jolie in Tomb Raider went a bit over the top with the poshness.
If it makes you feel better, my Mum can't do an American accent to save her life.

Hugh Laurie's accent in House is VERY weird. Brits tend to pull off the American accent quite well i think but Brad Pitt doing an Irish accent is a (audio) site to behold.

What about Anthony Hopkins in the movie The Worlds Fastest Indian. He was supposed to be Australian but he sounded as if he was from Bristol. The worst accent attempt ever. And Not up to Mr Hopkins regular standard.

Highlander - Frenchman playing a Scotsman with terrible accent, & Sean Connery playing Sean Connery making no attempt to do any kind of accent.

Denzel Washington in For Queen and Country had an accent that kept on crossing the Atlantic. This would have been bad enough but the story was about his character being British, not mid-Atlantic.
