The phrase is from John Donne's Mediations XVII :

No man is an island, entire of itself; every man is a piece of the continent, a part of the main. If a clod be washed away by the sea, Europe is the less, as well as if promontory were, as well as if a manor of thy friend's or of thine own were. Any man's death diminishes me, because I am involved in mankind; and therefore never send to know for whom the bell tolls; it tolls for thee.

The poet links up two themes here : one is the interconnectedness of all humanity, the other is mortality. Set in the context of the Renaissance and the Plague that pervaded Europe in those times, the paragraph deals with the tendency of some people or nations to isolate themselves from each other, and the fear of Death.

an island is piece of land surrounded by water all around the land and left alone in the middle of the ocean and no man is an island means that no one human is surrounded by any one as he has his own freedom and no man is alone in the world and cannot live alone as he can create his own world as he needs some people to create his own world
