The phrase is from John Donne's Mediations XVII :

No man is an island, entire of itself; every man is a piece of the continent, a part of the main. Any man's death diminishes me, because I am involved in mankind; and therefore never send to know for whom the bell tolls; it tolls for thee.

The poet links up two themes here : one is the interconnectedness of all humanity, the other is mortality. Set in the context of the Renaissance and the Plague that pervaded Europe in those times, the paragraph deals with the tendency of some people or nations to isolate themselves from each other, and the fear of Death.
