First wash it out in warm water and Vinegar, let the Vinegar sit for a while (about 5-10 minutes) to kill the germs. Use a Steel wool pad to clean it, that way if there are any scratches where some bacteria is stuck, it will help remove it.
Next let it soak in the sink with some baking soda and water for about 20 minutes, that will absorb the odor.
Then just wash it out with dish soap and water and the smell should be gone.
VIOLA!
p.s. Bleach will be too caustic too it, so I would avoid bleach.

Take a lemon cut it in half and place in lunch box overnight in the fridge, rub lemon all over it and rinse under tap.
Alternatively you could place in freezer over night, this also kills all germs that cause the smell.
My daughters lunch box always smells after a day at school ( left overs remaining till home time ), so everyday I have to do this. It does work.

BI-carb Soda- mix a couple of t-spoons with a little bit of warm water - let sit until the smell is gone and rinse clean

Put baking soda in it and close the lid.
Repeat if you have to.
Baking soda works like a charm. It's amazing!
Just sprinkle it all over the inside. When the odor is gone, just wash it out with dish detergeant and water.

Use ground or fresh coffee :
Rub onto all sides of the container, fill it up with water until about three-quarters full, cover with lid and give it a good shake. Open the lid and rinse with water. Repeat when necessary.

Have you tried spraying the inside with "Kaboom"? It's similar to "Fantastic" but works much better. It even takes the garlic smell out of tupperware that spaghetti sauce was stored in.

I changed my daughters lunchbox for one of those solid plastic ones that you can easily scrub clean. They are not that common these days but I found one in Toys R Us for a very cheap price.
Ok maybe it's not the trendiest lunchbox to have but it doesn't smell at all.
My daughter lets me off anyway... it has barbie on it. lol.
