Well, I've seen it and I've heard about it from one of my scottish friends who still wears a kilt. They wear nothing, no underwear, nothing under the kilt.

Tridition suggest they wear nothing at all, going natural. But today, some have come to wearing underwear of some sort. I'm not sure how common the underwears are, but I know they do now.

don't have a clue,never dated a Scottish guy..continually style of puzzled myself... different than to be completely truthful , I continually theory the kilts lead them to look sissified,,,, Sorry, if that offends absolutely everyone

Traditionally, nothing. Yup.
Many people nowadays are not so traditional. I'd have to imagine that in olden times... a strong breeze was rather notable.
