Australian movie called Wolf Creek.
Half the audience left during the opening as it was so scary.
I wouldn't want to backpack in that area EVER after seeing it.
It is WET YOUR PANTS scary, and based on a true story too.
I have never watched the whole movie, and I usually love a scary movie. It is too much for me.

John Carpenter's "In the Mouth of Madness" was pretty scary in a gory and mind bending sort of way. It's based on the works of HP Lovecraft and even though it was more of a sleeper movie, I've found it to be one of the creepiest movies I've ever watched.

George Sluizer's deeply disturbing psychological masterpiece The Vanishing (1988), which reveals the capacity for evil lurking beneath the most banal surfaces and the dangers of wanting to know too much.

There's an old creepy movie with George C. Scott called the "Changling" It's not to gory, but it has it's moments. Don't sell the old movies too short just because not enough blood and guts are not shown.
