Yes it can happen, but it's pretty rare. Sucks for the riders too. Think about being "stuck" upside down for a prolonged period of time.
My wife's cousin and her friend were on the cork-screw at Cedar Point in Sandusky, Ohio and it happened to them years ago. They were stuck upside down on the corkscrew part of the ride. They took them out with large platform lifts, 1 car at a time. She has a lifetime season pass to the park.

Interestingly enough, it has happened. If memory serves. it was Paramount Great America in Santa Clara, Calif. back in 1989 or 90.

i saw it on the news like that before. where it did stop upside down. it look damn scary. thats why i don't go on no rollercoaster that has a loop.
