I am assuming by criminal you mean felon. Bill Clinton was a felon for crimes committed while in office. He was impeached but not convicted by the Senate or removed as he should have been. He is the only President that I know of that has had a criminal conviction while in office but he only meets one part of your criteria. He was not appointed by the Supreme Court. As far as I know there has never been a President of the United States that was ever appointed by the Supreme Court.
So, I would have to say that the answer to your question is that it has never happened in the history of the US. I would assume it is possible to occur but it is highly doubtful. There has been a huge push on ethics in the last few years and it is doubtful that someone of Clinton's dubious character could get into the White House again. Character does matter irrespective of what the Democrats and Liberals would have us believe!!!!

The Supreme court has never appointed someone to the presidency. All presidents of the US have either been elected or appointed by the rules of succession.

He wasn't a criminal then, unless you count his DUI. He became a criminal when he started subverting the Constitution through illegal wiretaps and physical searches, collection of phone records, and ignoring the power of Congress through executive signing statements.
