Before you go asking people to fill out a survey that includes personal information such as name and address you really should provide some information that will assure the user this is legitimate. If this survey is for your dissertation why do you need to have personal identification information anyway? Aren't you just going to crush numbers with the information so the survey takers identity should be of no significance anyway. You should ask more generalized demographic information if you need things like what area of the country they come from and not their physical address and perhaps give them a number to differentiate the responses instead of asking for their name. Just my two cents worth, but I went to the site and then felt uncomfortable with the first two questions: name and address, so I didn't fill it out.
