It all depends on the energy level that the woman is enjoying at the moment.
It takes a skilled man to know how to find the pattern that works best for different women. Some women have the strongest orgasms from oral or manual stimulation. Sometimes penial stimutlation works best in a certain postition only.

in my experience, they get better and more intense. I think this is because there is more blood to the area.
