Should there be a formal format to initiate a recall, as there is for a Governor? Under what circumstances?

This would be by the voters themselves, not Congress as Impeachment.

Unfortunately, the US does not have this outlet. Other countries do, but here it can only be done for lesser officials. We can impeach but it is hard to find grounds and the Senate which handles impeachment is controlled by the Repubs.

I thought the whole notion of a Gubanatorial do-over in California was ridiculous and childish. I'd feel exactly the same if we enacted such a measure in regards to the executive branch.

We already have two fail safes: term limits and impeachment.

I think voters shouldn't elect him for another 4 years, but voters should vote him for not doing what a real President would do. I think he is unsatisfactory to the union, but also a huge @$$hole.

When a redneck piece of crap like George W. wanders around the world killing for profit and oil and then removes all the rights and freedoms granted under the Constitution from his own people then yes there should be a mechanism in place to remove the disease.

That would be nice. At least he can't be reelected. Write to your senators and congressmen and call for an impeachment. And a war crimes trial - I'd include Cheney and Rumsfeld in that!

A president must commit a serious grevience to be impeached. Saving your a$$ does not count as a serious grevience.
