My vote goes to The Hunchback of Notre Dame (complete with SUPER ESMERELDA....I'm suprised she didn't fly and have X-Ray Vision).

The Black Cauldron which is a shame because it was a good book.

I think every sequel they attempted to make was horrible. Cinderella 2 and 3. Lady and the Tramp 2, the second Bambi, the second Beauty and the Beast, Fantasia 2000-every single sequel they attempted to make was, in my opinion, the most horrible thing Disney could ever try to do.
I'd rather sit on shards of glass butt naked and poke my eyes out with hot coals.
Except for Toy Story 2, I like that one.

Some of the direct to DVD ones were pretty bad, but for the purposes of your question, I will stick to theatrical releases.

That said, Fantasia 2000 was pretty bad, as it had none of the charm and ground breaking originality of the first one.
