First, you should understand that the maturation of a hair happens in three cycles.
• The Anagen Cycle, or that phase in which the hair is actually growing;
• The Catagen Cycle, or the intermediate phase of hair growth when the lower two-thirds of the hair
follicle is destroyed
• And the Telogen Cycle, or that phase when hair growth is resting.
At any given time, about 85 to 90 percent of your scalp hair is in the anagen or growing phase. Less than 1 percent is in the catagen phase. And, the remaining 13 percent of the scalp hair is in the telogen or resting phase. Certain factors can rush the growing hair into the telogen phase prematurely, resulting in noticeably thin-looking hair and low hair density. Following the telogen phase, it takes about 3 weeks for the follicle to produce a new anagen hair.

Both references show diagrams of the skin that include hair follicles.
At the base of each follicle is a blood supply which brings materials to the growing base of the hair. It is only this part a hair that is living, the major part (the hair shaft) is just dead keratin. Our finger nails are dead keratin too which is just as well otherwise it would hurt when we had a hair cut or trimmed our nails.
Our entire skin is covered in hair follicles (except palms, soles, lips, etc,) but hair growth in different regions is quite different. It is very slow on legs for example, but faster on the scalp and fastest on an adult male's face.
The speed of growth is affected by all sorts of things. For example, a poor diet slows growth and growth more or less stops when a hair has reached a certain length (this can be confirmed by a girl with long hair).
From my own experience, the hair that remains on my scalp grows at about 3mm per week whilst my beard grows a little faster.

Hair is nothing but a collection of dead cells (somewhat like our nails). When more and more dead cells get deposited over the others, the length of the hair increases. This is how hair grows. In females hair grows faster& longer than males(I think). Hair also grows faster in summer & slower during winter.
