Staff was not terminated for any wrong doing, fault or stupid mistake. Just that could not complete the confirmation requirement. Please share your view or knowledge. Thank you.

Sort of depends on which confirmation requirement and why they couldn't complete it, don'tcha think?

You say the staff member's service was terminated? If that's the case, there will be a record. You also mention that it was more of a technical reason that the staff member could not complete the confirmation requirement. In that case, does the person now meet the confirmation requirement? If so, he or she can apply but will have to explain what happened the first time. If the hiring manager is sympathetic and what happened the last time doesn't put the person in any negative light, then it's possible for the person to be re-hired.
