It would vary with each case, but the individual would know. It should depend partly on what the parents intended when they named the individual and partly on what the individual prefers. This might clarify it, it could be Sally-Jane Ann Smith or Sally Jane-Ann Smith

unless they have a double barrelled first name (seperated with a hyphen) then all names between first and last are middle.
from you example, if it was Sally-Jane (first name) then Ann would be middle but with the lads name, you have 2 middle names there....
