85% of ankle sprains are inversion sprains. The ligaments effected are the anterior talofibular ligament, calcaneofibular ligament, and posterior talofibular ligament. The muscles effected may be the peroneus longus and brevis. The vascular, nervous, and lymphatic systems are also effected.

It wouldn't be the achilles tendon that would be affected first - it would be the lateral ligaments of the foot. Muscles that run on the lateral side of the leg and foot are also affected.

the achilles tendens are the most likely to be damaged as they are near the ankle and bear most of the weight.
