Every human being has got a name. Call her by her name. Except if you have some special relationship with your superior, that makes you feel uneasy to call the wife by her name or some Mrs. So and So. Even God corrected us when we call him names.

It depends on which country you live.
1. If USA than call her Ms. XYZ because they believes "Mrs." for aged lady.
2. If Europe call her Mrs. XYZ because they are always so keen for title.
(.........and when the boss is not around, try your luck : call her by name !!)

I would call her "Mrs. So-and-So" until she corrected me and asked me to call her something else (of course, I would use her last name........)
