Several ways:

1) Baseball fans pick the 8 starting position players through in-stadium and online balloting. In years when the game is played at an AL stadium, they also pick a designated hitter for the AL team.
2) A vote among players and coaches picks one additional player at each position, plus 5 starting pitchers and 3 relievers.
3) The manager of each team (who is the manager of the previous season's pennant-winning team) then picks players himself to get the roster up to 31 players per league. His picks, along with those picked by the fan and player ballots, must include at least one player from each team.
4) The last (32nd) player on each roster is selected by a final fan ballot from among 5 nominees.
