Most places do not numb it, it is rare that anyone will numb it even if they do it is only a local topical anistetic that only numbs the skin not the tissue so it still hurts. It hurts a bit but it's not that bad no more then an ear piercing. You should expect swelling and soreness for the first week.
The procedure is fairly simple they clean the outside area, and have you rinse with an antiseptic mouth wash. Then they mark the spot and then clamp the area (not all piercers use clamps).
After the area is clamped they piercer it with a hollow piercing needle, then remove the clamps. Then they will push out the needle with the jewelry, and screw on the ball.
Then it's done, that's it.

Honest to God answer.....
It did NOT hurt for more than 2 seconds! LITERALLY! As soon as the needle was thru, it went numb and was done with. I've had mine done for 3 years now. Never had any trouble with it.
I went right from getting the piercing, to dinner with co-workers.
Simple!
And no, they don't numb it, no reason to really.
A few friends of mine had theirs close up on them, it left a very tiny scar that unless pointed out, you'd never see it.
They have clear spacers to put in when you don't want it to be obvious.
I've had my tongue and eyebrow done also, they were very painful and caused a lot of pain in the long run. My labret piercing never did.
If you're this afraid of doing it, don't!
Good luck!

I have done it twice once when i was 18 and then i went to college and took it out and then did it again when i was 20.
it isn't the initial action of getting pierced that hurts. it feels strange and the shock of being pierced is odd. the hole wound hurts like a cankersore for a couple of weeks. you have to worry about your gums.. not your lower lip.. the rubbing of the backing next to your gums is what is the problem. and the hiding part.. if this isn't a good time to get a piercing in your life because you have to hide it, i suggest waiting until there is a time where you don't have to worry about it because when you first get pierced you don't want to take it out really.. it hurts.. and the piercer is required to give you bigger jewerly then you would like because of swelling. i am not sure if they have labret retainers ( clear plastic plugs) to hide your piercing i dont know if they make them for labrets. it will take about 4months to get used to and to know what length you really want and ball shape or size.

It will scar, they do not numb it, and if you are a male, then you could hide it with facial hair I suppose.
