First, you need to make sure you have indeed let them down. Most likely you have let yourself down more than them.
Secondly, just ask for forgivness. If they care, they will.
And I do.

Sincerity! To me it sounds like you feel bad for believing that you let someone down and I hope that is the case. It would indeed be a drag if you were worried about letting your boss down and now you might not have a job, but no matter the situation, it's ok to worry about the ramifications of your actions, but it is right to worry about how it made the other person feel.
Be sincere. Let them know what they mean to you, what the need you have is to let them know you are sorry. Don't make any excuses, even if there is a perfectly good one, it isn't about that. Be willing to accept the outcome of your apology even though it may be negative. A special person will recognize that you truly would like to set things straight even at the risk of losing your friendship and that is huge.
I'm sorry for your trouble. Hope things work out.

First of all, just come clean, take the blame and say you're sorry with no excuses. And then start going way out to show that you mean it, put up with the anger and accept it, keep being a friend/lover/family and helping out, and sooner than later, it will be behind you

The key is that people are more forgiving if you don't let them down often. If it's an isolated event, then they should let it go, unless it's unforgiveable or extreme.
