Do you mean modifications to put in an IEP? It really depends on individual student needs.

Commonly-needed modifications we use in the IEPs I attend include:
--seating to minimize distractions
--seating to provide proximity and/or line-of-sight to instruction
--shortened regular ed assignments
--modified assignments using simpler curriculum to address grade-level skills
--multi-sensory activities/instruction
--frequently re-direct student to task
--oral assessment of content-area knowledge
--re-teach using simpler, shorter steps
--single-step (or no more than two-step) directions
--use visual/pictoral supports for text
--provide text on audio (taped or peer-read)
--use manipulatives and other hands-on activities whenever possible

And so on. These should give you the idea. These modifications are applied across environments, regardless of whether in the regular or special ed environment.

I will try to give some ideas as much as possible, but if I knew more details on what you are needing to modify I could help better. Reading: you can let the students listen to the story on a tape, or read with another student. Worksheets: he/she could have someone help to read the questions, write the answers, or only answer part of the questions. Math: give them paper with grid squares to do each problem, only assign half of the work, allow them to use a hundreds chart, multipliation chart, counting blocks, or even a calculator (for checking only). Spelling: Cut down the number of words they have, have them practice with them daily, (abc order, sentences, 5 times each, syllables, etc.).
