What the party is for ("Lisa's 12th Birthday Party" for instance)
When the party is (date and time - when does it start and when does it end)
Where the party is (address)
Any special requests (dress up, bring bathing suit, etc)
If the guests have questions or need to RSVP (to tell you if they're coming or not), put down your phone number

Hmm, I'd say the normal stuff like:
-Who (whose b-day is it?)
-What (is it a party, b-day party, something else...)
-When (what's the date)
-Where (at your house, at a mall...)
-What to bring (if you're having a sleepover, bring a pillow & sleeping bag, or for a pool party, bring a bathing suit & towel)
