Head over to the Microsoft Office Live website - free hosting and a free domain paid for by Microsoft - aint it cool.

If you don't like the Office Live service (coz of the limited customization options) you could try dreamhost.com which has awesome packages.

I've yet to see a decent web hosting site that offers free or ad free hosting, let alone with a domain name. However there are some out there with a very limited availability.

usually the domain name will come free with any purchase of hosting.; the best deals out there are yahoos (around $11 and powweb hosting under $8)
powweb for the price is loaded with features and very easy to use. yahoo is even easier to use but a little more expensive, not sure exactly what youre looking for but you should do a side by side comparison to see what fits your needs the best. The domain price is irrelevant, its only once a year you have to pay it.

To host your site for free, I would highly recommend the following 2 sites.

Awardspace -> http://www.awardspace.com (PHP + MySQL)
50webs -> http://www.50webs.com

Both of these sites allow you to host for free and does not have any advertisement pop ups like those provided by geocities.

I have sites hosted on both the companies. Example

As for domain names, try to look out for domain name offers provided by Yahoo at only $2.99 for the first year.
