Ted's head looks like an orange stuck on a toothpick. His head is large and full of assorted alcoholic liquids.

It's because of his past alcoholism.
It's been proven that if a person drinks a lot, their nose grows to proportions that are not normal. Ted's face went through the same thing.

Ted's face has to relate back to his weight. He was always chunky as a child even (which is why he was called Teddy when his actually name is Edward) and his past issues with drinking and drugs don't help either.
He's losing alot of weight though, in case you didn't notice.

It's all a matter of perception. No matter what, work with what you got until you can change it, if you want to that is.
