Dear Ms. Boss;

Although I have enjoyed working with you at XYZ company for the past ?? years, I have decided to pursue other interests. Please consider this letter notice of my resignation effective (date).

Sincerely,
You

Best to keep it sweet and simple. Don't burn any bridges and for heaven's sake don't appologize for leaving that's silly. Best of luck in your new job.
