when is this medication safe for these patients and when not?
can a nurse refuse to give this medication to these patients?

Barring allergy, mannitol is a very safe drug. Any nurse who refuses to dispense a prescribed drug had best have a d a m n good reason. If said nurse worked under me as nursing supervisor, and did not have an exceptional explanation for refusing to carry out a legitimately prescribed medication order, that nurse would not only be dismissed immediately, but reported to the state board of professional disclipline. I've done that with two nurses, and both lost their licenses and had to pay a monetary fine. Then both were sued by the families of the patients whom the nurses deprived of their medications. It got real, real nasty and real, real expensive for those two. Take it for what it's worth, my dear.

If a stroke is caused by bleeding, medication (such as mannitol) can be given to reduce swelling of brain tissue.
A nurse is required to follow instructions and is obligated to discuss any objections regarding a doctors dosing instructions with the doctor.

Why are you asking people here that question. You should be asking a doctor, or at least a pharmacist. Don't trust many people here to know the answer, especially about medicine.
