Someone owes me over $10,000 and I can't afford a lawyer and have to do this myself. I have a signed and notorized promisary note from this person.

The Oklahoma courts system has a civil cover sheet on its website that is required with all civil case filings. However, the best resource for you will probably be either your local county clerk of court (if they even have a complaint form). If they have no forms available, go to the OK Courts site and you can follow the court rules for creating your own complaint. Also, if you want to investigate legal services for low income individuals at http://www.okbar.org/public/services/service.htm . Perhaps you qualify for free or reduced-fee legal services.
