I find the best way to eliminate stage fright is to take a public speaking class. If you don't have access to such a class, adopt someone. I mean, create a stage persona that is not afraid. You can do it until you overcome stagefright. Let this other persona take over on stage and you can do it.

There are two steps:
1) Be prepared. Practice by yourself, then in front of one or two friends or family members. Keep increasing the crowd if you can. You want to have as many positive experiences in a trusting environment before you perform in a more "scary" place. Try to pick an easy piece or something you know really well so you can have a series of "successes" to help build your confidence.
2) Perform as many places as you can. It also helps to practice in the place where you will be performing so you can get used to the environment.

Ignore the audience. If you are facing them look at the wall, the doors, something else rather than all the faces out there. Its easier if you are performing something like a dance or a theatrical scene where you can look at another performer on stage.
If you normally wear glasses or contacts, dont wear them on stage (of you can avoid this). If the faces in the audience are blurred they're not so scary.

the best way is to face your fear and be like you know what? i dont want this stupid stage fright anyway, so ima just go out there and do my thing and it will be great, even if no one likes it.
