A theory is something stated as a hypothesis. A theorem has most likely been proven or agreed to.

For example, a theory would be, "There are ponies in the center of the Earth." It may sound unreasonable, but is yet to be proven wrong or right.
A theorem, however, would be the Pythagorean theorem. It was once stated as a theory, but was soon proven with the right triangle.
